Student Enrollment 2014-2015
 
1.  Schedule your appointment to enroll your student(s):
 
Contact Central Enrollment at (717)566-5360 to schedule your appointment. 
 
Please provide the following information for each student when scheduling:
          
         - Parent or legal guardian email address
         - Last name, first name, middle name
         - Date of birth
         - Gender
         - Phone number
         - Address
         - Date moving into Lower Dauphin School District
         - Grade level for 14-15
         - IEP/504 education plan? (yes/no)
 
We will then prepare the Snapcode(s) you will need to complete the online portion of your enrollment, which can be emailed to you in advance of your appointment: 
 
2.  Enter your student(s) into the Infosnap database*:
 
 
*Note:  you will need your Snapcode to enter your student(s) into the Infosnap database; see required information above. 
 
(If you do not have access to a computer, please let us know when scheduling.  We will have a computer available for you to use, and will schedule an appropriate amount of time for your enrollment appointment.)
 
 
3.  Appointment to finalize enrollment:
 
Please bring the following documents to your enrollment appointment: (click HERE for details)
 
Required documents:
         - Proof of student age
         - Proof of residency in Lower Dauphin
         - Immunization records
 
Additional documents:
         - Last report card or transcript
         - Special education or Section 504 documents
         - Court orders or custody agreements
         - Foster placement letter (required if enrolling as a foster placement)
