Dear Parents/Guardians,

 

Welcome to the Lower Dauphin School District!

 

Enrollments are held by appointment only* on Tuesdays and Thursdays between the hours of 8:00 am and 3:00 pm at our District Administration Office, 291 East Main Street, Hummelstown, PA.  

Please have the following information available for each of your students when contacting us to schedule your enrollment appointment*:

 

-Last name, first name, middle name of student (as it appears on birth certificate)

-Date of birth

-Phone number

-Address

-Date moving in to Lower Dauphin School District

-Grade Level for 13-14

- IEP/504 education plan (Yes/No)


During your scheduled time, we will collect the required documentation (see below**), after which you will be creating an account and entering the data for each of your students directly into our on-line system, “InfoSnap”
.   Entering your data should take approximately 30 minutes for each student.  You will need to have a personal email address to create your family InfoSnap account.

**Please bring the following REQUIRED documents and/or information to your enrollment appointment in order to fully enroll your student(s). A detailed list can be found HERE:

-Proof of student’s age

-Proofs of residency in Lower Dauphin (2 forms)

-Immunization records

-Special education documents (if applicable)

-Court order/custody agreement (if applicable)

-Foster placement letter (if applicable)

-History of previous schools and dates attended
