Student Enrollment Procedure
2013-2014 students only
Schedule your appointment to enroll your student(s):

Contact Central Enrollment at (717)566-5360 OR via email to chelp@ldsd.org

We will need the following information from you when scheduling:

- Last name, first name, middle name of student (as it appears on  birth certificate) 

- Date of birth

- Phone number

- Address

- Date moving in to Lower Dauphin School District

- Grade Level for 13-14

- IEP/504 special education plan (Yes/No)

         *Note: You will need your personal email address to create your Infosnap account

          If a computer is not available to you, please let us know when you call to
          schedule your appointment; we will have a computer available for you to
          enter your information during your scheduled time.




Enrollment appointment:

Please bring the following to your scheduled enrollment appointment:

- Proof of student’s age

- Proof of residency in Lower Dauphin (1 form) OR notarized letter from landlord
              or home owner if residing with another resident

- Immunization records

- Special education documents (if applicable)

- Court order/custody agreement (if applicable)  Please note:  an Affidavit of
              Support (1302 Guardianship) will be required for students living with an
              adult other than parent or legal guardian)

             - Foster placement letter (if applicable)
