Students who have applied for lesson exemption must complete the following form each marking period. The form includes a section for lesson dates, private teacher evaluation, and a list of scales, exercises, solos, and/or etudes studied during the marking period. The form must be signed by parent, teacher, and student. Incomplete forms will not be accepted.
