PLEASE CONTACT US BEFORE PAYING FOR THIS TYPE OF SERVICE - WE WILL GLADLY SEND YOUR TRANSCRIPT FOR FREE.Transcript Request Form for current students - We require that students request transcript using a transcript request form and/or a request through Naviance. For students under 18 years of age, we also require a parent/guardian signature. Please allow 2 weeks to process college and scholarship applicationsTranscript Request Form for Lower Dauphin Alumni -- We require a request in writing from any alumni in need of a high school transcript. You can fax a transcript request form to: Danielle Taylor at fax# 717-566-3970 or send an email to her at email@example.com. Please indicate where the transcript should be mailed. We ask that you allow for 2 weeks processing time.Student Bio SheetThis form is often used by the School Counselor or teachers to write recommendation letters or press releases for students. You must log into Naviance to access your Bio Sheet so teachers/counselors can use the information to write a thorough letter of recommendation. Directions on how to access your STUDENT BIO SHEETAP Exam Form- Please be sure to submit application and fees to the School Counseling Office. Form may be downloaded when available. Deadline for AP test registration is TBD. 2020 testing will be held MAY 4-15. Forms will also be available in the counseling office.Young Student/Young Woman of the Month- this form is used by the Hummelstown Rotary and Women's Club of Hummelstown to complete press releases for local news publications. Senior students receive these forms from the Counseling office when they have been selected for of these awards.
