Clearance Directions


-VOLUNTEER REQUIREMENTS-


 
Thank you for choosing to volunteer at Lower Dauphin. Below are the requirements you must complete before you are permitted to volunteer. Once you complete these requirements, you will be permitted to volunteer throughout the district and receive an approved Volunteer badge. This badge will also have an expiration date, which will indicate the date you will be required to present new clearances to the District Office in order to continue volunteering.
 
  • State Police Criminal History Record – Act 34 (Dated within the last 5 years)
  • Federal Criminal History Background Check – Act 114 (Dated within the last 5 years)
    • OR Affidavit if you have lived in Pennsylvania for ten or more years
  • Pennsylvania Child Abuse History Clearance – Act 151 (Dated within the last 5 years)
  • TB test results (Results must read negative and be dated within the last 90 days)

Print and complete the Volunteer Info Coversheet 2023 Update to hand in with your other required clearances. This form can also be used as a checklist for all of the requirements and will assist us in getting your Volunteer badge approved.

 

 


-ACT 34 - STATE POLICE CRIMINIAL HISTORY RECORD CLEARANCE-


The Pennsylvania State Police has established a web-based computer application called “Pennsylvania Access To Criminal History” (PATCH). Using this system, a requestor can apply for a PA criminal background check online. This clearance is valid for 5 years from the date of issuance. “No Record” certificates are returned immediately 85% of the time, via email. There is no cost for a Volunteer.

Pennsylvania Access to Criminal History (PATCH) Website https://epatch.pa.gov/home 

  • You will need your Social Security Number.
  • Log in to the website listed above, then select “New Record Check” (VOLUNTEERS ONLY).

  • Select “Accept” and then complete the required fields with your personal information.

    • NOTE: Reason for Request should already be filled out, by the computer, and say "Volunteer FREE".
  • Complete the required fields with your personal information as requested.

  • Select "Enter This Request”.
  • Once you get your notification email, to let you know your clearance is complete, click on the blue control # beginning with “R”. Click on the blue “Certification Form” to get the official State Police clearance.

    • Please immediately print out your official record or save it as a .pdf, as you will not be able to go back to this page!
  • If your status indicates “Under Review”, record your reference number and date of application. Access the website periodically to check the status of your record. Select “Check the Status of a Record Check”. Complete requested information EXACTLY as you had with the original application. If your status has been updated to “No Record”, select the “Certification Form” link and print the official record, with the state shield in the background.

  • If your clearance has a record attached to it, you will need to provide the additional information sheets, added by PATCH, for review by the Director of Human Resources.

 

 


-ACT 151 - PENNSYLVANIA CHILD ABUSE HISTORY CLEARANCE-


 As of January 5, 2015 you may now gain the PA Child Abuse History Clearance online. This form is distributed and processed by the PA Department of Human Services. There is no cost. (If you have previously applied for a Child Abuse clearance and are applying again within the five year time limit you may be charged.) Pennsylvania Child Welfare Portal recommends that you do not use Chrome Browser to obtain this as they have been having issues with this browser recently.

  • You will need your Social Security Number.
  • Select the “Create Individual Account” and then select “Next”.

  • Complete required information and then select “Finish".
    • NOTE: Your Keystone ID is a username that you create and must have 6 to 10 characters, 1 upper case, 1 lower case, and one symbol.

    • Be sure to write down your username, password, and security questions/answers. You will need these again to log in for your clearance results!
  • You will be e-mailed your temporary password to the e-mail account you provided, previously, in the required information.

    • You will need to close the window that you created your Child Welfare Portal account in and reopen a new window to proceed with your Keystone ID (username) and your temporary password (sent to you via e-mail).
  • Go to the https://www.compass.state.pa.us/CWIS website, and select “Log In”.

    • Select “Access My Clearances” and then select “Continue” at the bottom of the page.

    • Enter your Keystone ID (username) and your temporary password (sent to you via e-mail).

  • You will then need to change your temporary password to a password you select, then select “Submit”.
  • Log out again.

  • Go to https://www.compass.state.pa.us/CWIS website, and select “Log In”.
    • Enter your Keystone ID (username) and your password you just created.

    • I have read…” and then select “Next”.

    • Select “Continue” at the bottom of the page and then select “Create Clearance Application”.

    • Select “Begin” and then select “Volunteer”.

    • Continue completing Part 1 and Part 2 of the Clearance Application.

  • You should get an email notification when your clearance has been completed (as well as a copy mailed to your house if you selected that).

 

 


-ACT 114 - FEDERAL CRIMINAL HISTORY BACKGROUND CHECK (FBI) OR Affidavit-


 If you have lived in Pennsylvania for 10 years or more, please complete the Affidavit in lieu of the Federal Criminal History Background Check in accordance with Act 153.

If you have not lived in PA for 10 years or more, you will be required to complete the FBI Clearance. This criminal history check is also known as your FBI Clearance or FBI Fingerprints Clearance. Volunteer clearances are valid for 5 years from the date of issuance.

PA Department of Human Services (DHS) which indicates all individuals governed by the Child Protective Services Law (CPSL) must obtain clearances through DHS, which includes volunteers that volunteer in public, private and nonpublic schools. To obtain a Volunteer Service Code, individuals must contact DHS directly: https://www.dhs.pa.gov. 

You must register online at https://uenroll.identogo.com prior to going to the fingerprint site. There is an estimated cost of $26.55 per request. We do not have an authorization code as we do not reimburse you for completing this clearance.

To complete your pre-enrollment registration online, go to https://uenroll.identogo.com

  • Choose “Schedule or Manage Appointment” and then select “Next”.

  • Enter personal info required and then select “Next”.
  • Create a security question, enter the answer, and then select “Next”.

  • Answer citizenship questions and then select “Next”.

  • Answer personal questions required and then select “Next”.

  • Enter personal info required and then select “Next”.
  • Enter more personal info required and then select “Next”.

  • Enter your address info and then select “Next”.

  • Choose from a drop down box which identification document you would like to bring along for the fingerprinting. (Ie; License, passport, etc.), select “Yes” if the name on the document matches your current name, and then select “Next”.

  • Enter your zipcode, search for the closest fingerprint locations, and select the location of your choice.

  • Select a date and time from the drop boxes. Or check “Walk-in” and then select “Submit”.

    • NOTE: All scheduled appointments take precedence over “walk-ins”.
  • You will then see a message that says, “You have successfully Pre-Enrolled.” You will also receive a follow-up e-mail stating the same thing.

 Special Covid-19 Note

Due to the impact of the Covid-19 epidemic they are experiencing a temporary reduction in the number of fingerprinting locations. Fingerprinting staff and customers are required to wear face coverings to enter fingerprinting locations unless exempted due to age or a medical condition.

-TB TEST-


A TB test is required in order to volunteer.

  • Note:  As long as you keep your clearances current you will only need to supply one TB test to us, which won't expire.

  • Must be completed within the last 90 days (when submitting your other clearances) in order to be valid.

 

 


-UPON COMPLETION-


 
After you have gathered all necessary required documents, please email them directly to Melissa Fullerton OR bring copies of each document to the District Administrative Center [291 East Main Street, Hummelstown] or any school office.
 
Your documents will be reviewed by Human Resources and once they are approved, you will be contacted via e-mail about where you can pick up your approved Volunteer badge.
